Chris Hoyler 43rd Airlift Wing Public Affairs POPE AIR FORCE BASE, N.C. -- The 43rd Medical Group will begin processing Preventive Health Assessment's with a Web-based system Feb. 1. This system, which is being implemented throughout the Air Force, will allow 43rd MDG personnel to review health needs of patients through a web-based health questionnaire. In addition, patients' records are now being reviewed by medical personnel without the patient being present, as long as the patient is not on flying status. No follow-up appointment is required in most instances, but if other tests or procedures are needed the member will be contacted. "The Web-based PHA was created to benefit all members who may be geographically separated from their unit, such as those on deployments or in the (Air Force) Reserves," Staff Sgt. Nicole Mason, 43rd MDG PHA technician, said. The process begins with the patient receiving a notice that they are due for a PHA. The questionnaire must be completed prior to the patient's due date, and it can be accessed from any Web-enabled computer at home or work. The patient goes to http://www.defenserack.com and selects the AF Web Health Assessment option. From there, they select "patient" and enters their Social Security number and date of birth to log in to the system. Patients can start and stop the questionnaire as many times as needed and it will always revert back to the incomplete areas upon log in. When the questionnaire is completed, the patient needs to print a personal report for themselves. The whole process takes about 15 to 20 minutes, according to Sergeant Mason. "After we have received their assessment members will receive a call or email from us here in the PHA clinic to notify them if labs, immunizations, audiograms or occupational health exams are needed," Sergeant Mason said. If patients have concerns about their health they are encouraged to make an appointment with their Primary Care Manager or come in during sick call. Although the online system will make things easier, there are always exceptions to ensure that everyone is receiving the best care possible. "Members will still need to call the appointment line to schedule appointments with the PHA clinic for retraining physicals, separation physicals, special duty clearances and overseas clearances," Sergeant Mason said.
